02, remember to bring your OBS form. Get it signed by a doctor. A legally certified doctor. Not a quack. You'll be able to tell when you talk to him.
this is the scenario of a real doctor talking:
someone from 02: hello doctor, i would like to get a body checkup.
doctor: hello, come in and sit down.
and this is the quack:
someone from 02: hello doctor, i would like to get a body checkup.
quack: QUACK QUACK!!!
See the difference?
Don't forget to sign edusave form if you want to use it to pay for OBS. Remember to bring health booklet too.
